Doctor-patient relationship is the key

With the Humana Group Medicare HMO plan, you work one-on-one with a primary care physician (PCP) who coordinates your care and provides referrals to other in-network providers when necessary.* Plus, you aren't locked in once you choose a PCP – you can switch to another in-network doctor who is accepting new patients anytime.

You also have access to a variety of programs and services that promote healthy living and aging – at no additional cost. Value-added features include SilverSneakers^® Fitness Programs (in most markets) and Humana Active Outlook^SM.
