You must continue to pay your Medicare applicable premiums if not otherwise paid for under Medicaid or by another third-party.

You must use plan providers or those who accept Humana’s payment terms and conditions. You must use network pharmacies for Humana Part D coverage. You will pay more for covered services received outside Humana’s networks.
